Search for your name and file a claim online through the claim site the Taxation and Revenue Department links from its File a Claim page, then send the proof of ownership TRD asks for.
No one may sign you up before the 1461st day after the money is handed to the state.
Any deal must be in writing.
You never need to pay anyone to file.

Steps
Search for your name through the Taxation and Revenue Department's Search Unclaimed Property page.
Create a claimant account and file the claim
Provide the proof requested, including a signed and notarized claim form when required
TRD has 90 days to approve or deny a claim and 30 more days to pay it

Signed and notarized claim form
Copy of government-issued photo ID
Proof of Social Security number
Proof you lived at or received mail at the listed address
For a deceased owner: death certificate and proof of entitlement under heirship law
