Documents
Signed claim form if one is generated (notarized only for claims of $5,000 or more)
Proof of address or ownership if asked
For a deceased owner: relationship affidavit or short certificate
What it costs you
free
If someone offers to do it for you
No one may charge you more than 15%.
Any deal must be in writing and notarised.
You never need to pay anyone to file.
